Facial Injury Severity Scale

A point score for craniomaxillofacial trauma, summed across the mandible, midface, upper face, and soft tissue. The result updates as you type.

Mandible

2 points each

1 point each

Midface

1 point each side

Upper Face

1 point each

Soft Tissue

1 point each

Method

FISS = sum of points for each fracture and laceration

Points are assigned by anatomic region and weighted by the complexity of each injury: Le Fort III and displaced frontal sinus fractures score highest. Each mandibular, zygomaticomaxillary, and orbital fracture is counted separately. There is no maximum score and no fixed severity cutoffs.

In the development study of adults with facial fractures, higher scores were associated with longer hospital stays and higher hospital charges. The scale was developed in adults, and pediatric fracture patterns differ.
